Wide QRS complex tachycardia in a patient with Wolff-Parkinson-White syndrome and cardiomyopathy: what is the mechanism?
نویسندگان
چکیده
A 32-year old woman with severe mitral regurgitation. congestive heart failure, and Wolff-ParkinsonWhite syndrome developed frequent, nonsustained episodes of a wide QRS complex tachycardia several days postpartum. Figure 1 shows an ECG with telemetry lead I and modified chest lead (MCL) recorded while the patient complained ot" palpitations. The same pattern of initiation and termination of the wide QRS complex tachycardia was seen repeatedly. An example ot" ventricular preexcitation during sinus rhythm is shown in an ECG with identical leads in Eigure 2. What is the mechanism of the wide QRS complex tachycardia?

Wide complex tachycardia in a patient with pre-excitation: What is the mechanism?
Introduction Patients with Wolff-Parkinson-White syndrome are generally evaluated for symptomatic supraventricular tachycardia (SVT). Not infrequently, these patients are also at risk of wide complex tachycardia. Although overt pre-excitation in sinus rhythm is most often present in patients with WolffParkinson-White syndrome, the absence of a delta wave on the resting electrocardiogram (ECG) d...
متن کاملDevelopment of rapid atrial fibrillation with a wide QRS complex after neostigmine in a patient with intermittent Wolff-Parkinson-White syndrome.
We report the case of a 67-yr-old man with intermittent Wolff-Parkinson-White (WPW) syndrome in whom neostigmine produced life-threatening tachyarrhythmias. The patient was scheduled for microsurgery for a laryngeal tumour. When he arrived in the operating room, the electrocardiogram showed normal sinus rhythm with a rate of 82 beat min-1 and a narrow QRS complex which remained normal throughou...
متن کاملVentricular Tachyeardia during Cardiac Cathe - terization of Patient with Wolff - Parkinson - White Syndrome
T HE Wolff-Parkinson-White syndrome consists of an abnormally short P-R interval, usually prolonged duration of QRS complexes, and history of repeated episodes of paroxysmal tachycardia.' Variations of QRS complex have been observed during tachycardia, following exercise or atropine sulfate,1 or even while patients were at rest.2 The paroxysmal tachyeardias have been generally supraventricular,...
متن کاملLate onset of Wolff-Parkinson-White syndrome in a 72-year-old man.
We encountered a case of wide QRS tachycardia with chronic atrial fibrillation in Wolff-Parkinson-White syndrome. Unique features were late onset of syncope attacks associated with this tachycardia at an advanced age of 72 years old without previous documentation of Wolff-Parkinson-White syndrome on electrocardiogram. He had a high likelihood of sudden cardiac death. Catheter ablation using CAR...
متن کاملAtrial flutter with 1:1 conduction in undiagnosed Wolff-Parkinson-White syndrome.
BACKGROUND Atrial flutter with 1:1 atrioventricular conduction via an accessory pathway is an uncommon presentation of Wolff-Parkinson-White syndrome not previously reported in the emergency medicine literature. Wolff-Parkinson-White syndrome, a form of ventricular preexcitation sometimes initially seen and diagnosed in the emergency department (ED), can present with varied tachydysrhythmias fo...
